Financial & Appointment Policies

Payment Policies
Your insurance copayments and deductibles are due at the time of service. Before your appointment, our staff will verify your insurance coverage and help explain any potential out-of-pocket costs.

When required by your insurance provider, we will obtain prior authorization for your services.
Appointment Guidelines
All visits at Valley Renal Specialist are by appointment only. If you're visiting us for the first time, we encourage you to download and complete the required forms before your visit.

If you're unable to complete the forms in advance, please arrive 15 minutes early to complete them in our office. As a courtesy to our patients, we provide reminder calls for all upcoming appointments.
Cancellation Policy
We understand that circumstances may require you to change your appointment. For your convenience, we maintain a 24-hour phone message system where you can cancel or reschedule appointments you're unable to keep.

Please note that cancellations made less than 24 hours before your scheduled appointment are considered short notice, as this makes it difficult to schedule another patient in your reserved time slot. A $50.00 fee will be charged for missed appointments without prior notification or cancellations with less than 24 hours' notice.

This policy helps us maintain efficient scheduling and ensure appointment availability for all patients who need care.
