When required by your insurance provider, we will obtain prior authorization for your services.
If you're unable to complete the forms in advance, please arrive 15 minutes early to complete them in our office. As a courtesy to our patients, we provide reminder calls for all upcoming appointments.
Please note that cancellations made less than 24 hours before your scheduled appointment are considered short notice, as this makes it difficult to schedule another patient in your reserved time slot. A $50.00 fee will be charged for missed appointments without prior notification or cancellations with less than 24 hours' notice.
